Jeremiah (JV) 40 passed peacefully on December 30, 2020 at St. Vincent's Hospital. Beloved son of William Swartz (Cathy Forsith) and the late Tina Swartz (nee Simms); cherished sister of Emily, David (Sharon), Patrick (Traci), and Thomas Swartz.
Further arrangements are pending. Jeremiah will be laid to rest at Sunset Memorial Park.
In lieu of flowers, memorial contributions may be made to his family.
