Chad Polk was born in Houston, Texas to Josephine Polk and Jessie Polk. He graduated from Lee High School where he meet his wife and later had two lovely kids. He is survived by his wife Kim; kids Timothy and Lanisha Polk. He was a wonderful father, husband, brother, son and friend. Chad was a gift from God to me and now the Lord has called him home. He will be in Heaven with the rest of our love ones that have passed from this virus. Come and celebrate his life with us!
