CROMWELL — Chelsea L. Jacobs, 25, of Cromwell died suddenly Saturday, August 22, 2015, at IU Methodist Hospital in Indianapolis. She was born on June 23, 1990, to Michael and Kim (Herrick) Johnson. She was married to Shelby Jacobs Jr. She loved to spend time with her family and enjoyed playing video games. She is survived by her husband, Shelby Jacobs Jr. of Cromwell; newborn daughter, Mila Grace Jacobs who was born on August 16, 2015; her mother, Kim Johnson of Syracuse; her brothers, David (Sueann) Julian Jr. of South Whitley, Bobby Herrick of Syracuse, and Alan Daub of Georgia; and a sister, Bethany Moore of South Whitley. Also surviving are her maternal grandparents, Lillian Brosseau and Michael Herrick; her paternal grandparents, Charles and Brenda Johnson; her father and mother-in-law, Shelby Sr. and Stella Jacobs; four aunts; many cousins, and nieces and nephews. She was preceded in death by her father, Michael Johnson. Funeral services are at 11 a.m. Saturday, August 29, 2015, at Shiloh Baptist Church, 709 Johnson St., Ligonier, with visitation one hour prior to service. Visitation also from 4-8 p.m. Friday, August 28, 2015, at the church. Burial at Rose Hill Cemetery, Albion. Memorials are to the family.
